Leland Cole has been under my care for multiple musculoskeletal problems since 1980. He was disabled by knee pain and underwent arthroscopy in November of 1980 which did not resolve his symptoms or complaints which were principally those of a weak left leg with muscle atrophy measured at 1” with accompanying fatigue and limping. An extensive evaluation including EMGs, x-rays, further diagnostic studies including a muscle biopsy did not identify any significant pathology. Based upon my review of the records and current examination of the patient, I have identified that he was substantially disabled form 1980 through 1985 but that at the present time his recovery appears quite complete. I recognize a slight difference in rotary alignment of his hips but this is not of clinical significance. He has regained maximal strength in both legs and is at this time fully conditioned and recovered.
